Let me tell you that how I built it.
First of all what you have to do is just open a new tab on your browser and search Jotform. On the right hand side, click on Sign up, then sign up with you E-mail ID and password.
On the right hand side you would see a button named as "Create From", click on it. The choose and customize your contact us from according to you.
